Who pays for treatment after a car accident in Ontario
Every Ontario auto insurance policy includes accident benefits that pay for medical and rehabilitation treatment after a collision, whoever was at fault. You claim them from your own insurer. Passengers, pedestrians and cyclists are usually covered too, through their own policy or another policy involved in the accident.
Which insurer pays first depends on the date of the auto policy:
- Policies issued or renewed on or after July 1, 2026. The auto insurer pays first for eligible treatment such as physiotherapy, chiropractic care and massage therapy. Your work benefits are kept for other care. Medication is the exception and still goes through your health plan first.
- Policies issued or renewed before that date. Your extended health plan, if you have one, is used first, and the auto insurer pays what is left.
We check which rule applies to you at your first visit. In both cases the insurer reviews the treatment plan and approves the care it considers reasonable and necessary.

What to do in the first days after a collision
- Get urgent symptoms checked first. Go to the emergency department or call 911 for a severe or worsening headache, confusion, repeated vomiting, fainting, chest pain, trouble breathing, weakness or numbness in the arms or legs, or loss of bladder or bowel control.
- Tell your insurer. Insurers ask to be told about an accident within seven days, and to receive your completed application within 30 days of sending you the forms. Your insurer or broker can confirm the dates that apply to you.
- Be assessed early, even if the pain seems mild. Symptoms often build over the first few days.
- Keep your paperwork together: the claim number, the adjuster's name and anything you have been sent.
