This website is operated by Well Care Medical, a rehabilitation and wellness clinic at 33 Holland St E, Bradford West Gwillimbury, ON L3Z 2B8, Canada, which uses the name WellCare Medical. In these terms, "we", "us" and "our" refer to Well Care Medical.

The content on this website is provided for general information. It is not medical advice, diagnosis or treatment, and it does not replace an assessment by a qualified health professional. Do not delay seeking care, or disregard professional advice, because of something you read here. Using this website or contacting us through it does not create a clinician-patient relationship. In an emergency, call 911 or go to the nearest emergency department.

Appointments are booked by phone or email and are confirmed by our front desk. A message sent through the website is a request and is not a confirmed appointment until we reply. If you cannot attend, we ask for at least 48 hours' notice so the time can be offered to someone else. A $50 fee applies to missed appointments and to cancellations made with less than 48 hours' notice.

Fees are payable at the time of your visit unless we are billing your insurer directly. Direct billing is offered as a convenience and depends on your insurer accepting the claim. Coverage is decided by your insurance plan and not by us. You are responsible for any amount your plan does not pay, including deductibles, co-payments, amounts above plan limits and declined claims.

Email and website forms are not fully secure methods of communication. Please do not send detailed personal health information through the contact form. Use it for general questions and appointment requests, and discuss health details with your clinician.

We collect, use and protect personal information as described in our privacy policy, which forms part of these terms.

We work to keep the information on this website accurate and up to date, including our hours, services and the list of insurers we bill. Details can change, and we do not guarantee that the site is free of errors. Please call (365) 247-0440 to confirm anything you are relying on.
